Remember a loved one by making a gift commemorating their life or honor a special person by expressing your appreciation of special support, care or friendship. You may designate your gift to support specific Evergreen Healthcare programs or services.
A special gift card announcing your gift may be sent to whomever you wish as a way of letting a family member or friend know that they are in your thoughts. The amount of your gift will not be indicated.
Memorial Donations In Memory of A Loved One
If you would like to designate the Evergreen Hospice Services to receive donations in memory of your loved one, we suggest the following verbiage in the obituary: “In lieu of flowers, please make a donation to Evergreen Healthcare Foundation (MS-5), Hospice Services, 12040 NE 128th Street, Kirkland, WA, 98034”.
The Foundation can also provide you with donation envelopes to distribute to families and friends. Please contact the Foundation at 425.899.1900 for more information.
Your family will be notified of donations in memory of your loved one as soon as a gift is received. A thank you card for each donor will also be provided for your convenience.
