The Evergreen Surgery and Physician's Center is home to Evergreen's Education Center and three floors of physician office space.
The top (5th) floor of the Center is the Evergreen Surgery Center, with it's own entrance from the top floor of the Blue parking garage.

DIRECTIONS
Northbound on 1-405
- Take the NE 124th Street/Totem Lake Boulevard Exit (20B); stay to the right
- and follow the Totem Lake Boulevard Exit.
- Proceed straight from the stoplight, up the hill to NE 128th Street and take a right at the stoplight.
- Drive past the main entrance to the hospital and continue on the road as it curves north and becomes 124th Avenue NE.
- When you come to the stop sign, turn left into the Blue Parking Garage (the parking garage entrance is located across the street from the Evergreen Hospice Center).
Southbound on 1-405
- Take the NE 124th Street Exit (20); turn left onto 124th Street.
- Take a left at Totem Lake Boulevard/124th Avenue NE.
- Proceed to 120th Avenue NE and take a right; go up the hill to NE 128th Street and take a right at the stoplight.
- Drive past the main entrance to the hospital and continue on the road as it curves north and becomes 124th Avenue NE.
- When you come to the stop sign, turn left into the Blue Parking Garage (the parking garage entrance is located across the street from the Evergreen Hospice Center).
