Evergreen Surgical Center is located on the 5th floor of the Evergreen Surgery and Physicians Center, adjacent to Evergreen Hospital Medical Center. The entrance is located on the top floor of the Blue parking garage, with special parking for surgery patients.

Directions
Northbound
From I-405 north, take the NE 124th St./Totem Lake Boulevard exit (#20-B). Stay in the right lane and take the Totem Lake Boulevard sub-exit. Go straight through the light at the end of the exit. This street will take you between the upper and lower levels of Totem Lake Mall. Proceed to the top of the hill to the light at NE 128th Street. Turn right and follow the signs to the upper level of the parking garage (located across the street from the Evergreen Hospice Center).
Southbound
From I-405 south, take the NE 124th St. exit (#20). Turn right at the end of the exit onto NE 124th.
Turn right at the next light, onto 116th Way NE. Turn right at the light onto NE 132nd, and right at the light onto 124th NE. Follow the signs to the upper level of parking garage (located across the street from the Evergreen Hospice Center).
