For all individuals who are offered and accept a position at Evergreen Healthcare, we provide the following to help you prepare for your start with Evergreen.
Required Forms for newly hired employees to complete:
All offers of employment are contingent upon successful completion of the New Hire Credentialing Process. This includes completion of the following documents, a drug screen and a criminal background check.
- Employment Application - completed online when you apply for a specific position
- Disclosure statement for criminal history
- Satisfactory reference checks
- Verification of education and experience
- Department of Health: Health Professions Quality Assurance Search
- Office of the Inspector General (OIG) Fraud and Abuse Search
All forms will need to be completed and submitted to Human Resources no less than five days before your start date with Evergreen Healthcare:

Other forms and documents that you will be required to provide to Human Resources or complete between the time of a formal job offer and the end of your first day will include:
- ID to satisfy I-9 requirements (i.e., driver's license AND Social Security card, driver's license AND birth certificate, or passport by itself); for a more complete list of acceptable documentation, click here.
- Check or deposit slip from bank or credit union to establish direct deposit
- Current National/Washington State Healthcare License (if applicable)
- Current Healthcare Provider CPR Card (if applicable)
- Proof of other credentials/certifications
- Current contact information for new employee
- Current emergency contact information
- Immunization records to document TB, Hep B and MMR. Each new employee will meet with Employee Health to determine immunization status and requirements prior to their first day of employment.
- Approval and release for background check and drug screen. Each new employee will successfully pass a criminal history check and drug screen before beginning employment.
- Current W-4 for tax purposes
