Evergreen Horizons
Evergreen Horizons provides free or low-cost health education classes and screenings at area senior centers and assisted living facilities as well as on the Evergreen campus. Horizons provided classes to 2000 seniors in 2005.
Evergreen Senior Clinical Services
This partnership with various senior, wellness and adult day centers in the community targets at-risk seniors in the community with health promotion outreach, dental care and health enhancement services. More than 400 seniors were served during 2005.
Senior Health Specialists
The Senior Health Specialists is a specialty clinic where geriatricians provide primary care services to older adults, with expertise in handling the complex medical challenges of seniors. The program handled more than 16,000 patient visits in 2005.
