Whenever special needs arise as a result of aging, illness or disability, Evergreen Care Network (ECN) social workers and nurses create a network of support from the many social and health services that exist in the community, enabling people to live as independently as possible.
Highlights
ECN continued to partner with internal Evergreen programs (hospital, home health, In Home Mental Health) as well as the broader community (Overlake Hospital, ADS, Home Care Agencies, mental health agencies and senior centers).
ECN’s ongoing client assistance contributed to individual client’s well being as well as benefits the overall community by providing quality in home case management allowing elders and disabled persons to remain in their own homes and not rely on more expensive, taxpayer funded residential care.
ECN provided almost 10,000 client months of services in 2005.
