Community Health Education
Our programs are designed to enhance all aspects of a healthy community. Classes run the gamut from parenting and safety education to meditation, exercise and the management of chronic health conditions. Evergreen Healthline
This free service provides 24/7 telephone access to consulting nurses, physician referrals and registration for Evergreen health classes and seminars. The Healthline handled more than 200,000 calls in 2005.
Diabetes Education
This is an outpatient education program for people diagnosed with Diabetes. Offers a series of classes that covers everything you need to know about managing diabetes. This program served more than 160 patients though a series of self care modules that covers medication and diet management, exercise, stress management and lifestyle changes.
Grief and Bereavement Services
The Grief and Bereavement program provides compassionate support for family members. Levy funds help underwrite care for those who cannot afford it. The program served 997 patients in 2005.
Booth Gardner Parkinson's Care Center
The region’s only full service evaluation and treatment program for Parkinson’s disease and other movement disorders. The Center served more than 1500 patients in 2005.
Community Health Status
This program brings together Evergreen services and a variety of outside partners to tackle community health concerns, such as 2005’s flu shot campaign.
