Prior to your first visit to the Senior Health Specialists, please read this Welcome Letter (PDF) and then print and complete the entire Pre-Visit Questionnaire (PDF). This will save time, and allow us to spend the valuable time with you in your appointment and not filling out paperwork.

Along with the completed Pre-Visit Questionnaire, please bring the following items to your initial appointment:
- All medications that you are taking (including vitamins, herbal and other non-prescriptive remedies)
- Insurance information
- Eye glasses and hearing aides (if you have them)
- Old medical records
- Advanced directives (if you have them)
