HOME HEALTH CARE
Home health services provide many specialized services to people who are ill or disabled and living at home. Services include: skilled nursing care, medical social services, personal care, and physical, occupational and speech therapy. Medicare, Medicaid, and private insurance may pay for most of the service. In order to have Medicare, Medicaid or private insurance pay, a physician must authorize the service and it must be provided by a Medicare/Medicaid certified agency.

Remember: “home health services” generally include specialized services related to medical care, while “chore services” provide help with daily living and household tasks.

RESPITE CARE
The Respite Program offers relief for caregivers who provide full-time care for an elderly or disabled person and are unpaid for their work. Respite care may be provided in a variety of settings – at home, in an adult day center, or nursing home. The care is paid for on a sliding fee scale, based on the income of the person receiving care. Respite may be used occasionally or at regular intervals.

PRIVATE CONSULTATION AND CASE MANAGEMENT SERVICES
Private Case Managers are trained social workers and nurses who provide tailored services to seniors and their families who need to make life changes, investigate services for the home, consider moving and/or need ongoing visits. HOSPICE PROGRAMS
Hospice programs and services offer care to terminally ill persons and their families. Care can be given on an inpatient basis or in the patient’s home. Hospice care focuses on the emotional, social and spiritual needs of a dying person and that person’s family, as well as the patient’s physical and medical needs. Emphasis is on pain control and symptom management. These hospice programs are located in East King County.

Support and care for persons facing a life threatening illness with a limited life expectancy, usually around six months. Evergreen provides support across all settings, from the patient’s home, to the nursing home, to the Evergreen Hospice inpatient center. Hospice offers services such as nursing, medical direction, medical social work, counseling and bereavement as needed. Accepts Medicare, Medicaid and insurance. King and Snohomish Counties.
