Evergreen Medical Group, Sammamish
Vaccine supply is limited, and will be given on
a first-come basis for the following groups:
Ages 6 months to 35 months
Pregnant women
Parents and caregivers of children under 6 months
Ages 3-24 years
Patients with chronic conditions that put them
at risk for influenza complications
*You must be a patient of an Evergreen primary care center, which includes our six Evergreen Medical Group sites and Family Medicine of Redmond.
The vaccine is free.
There will be a $15.60 administration fee.
