Monique Giroux, MD
Dr. Giroux is the medical director of the Parkinson’s Care Center. She completed a two-year fellowship in movement disorders at Emory University in Atlanta and is board certified in neurology. She is also medical director for the National Parkinson Foundation Center of Excellence for Parkinson’s disease. She has participated in numerous clinical trials, including gene therapy for PD. The focus of her work in care and research is on the development of interdisciplinary rehabilitation, team medicine and wellness for PD. Curriculum Vitae

Alida Griffith, MD
Dr. Griffith is a Movement Disorders neurologist, fellowship-trained at Harvard-affiliated Beth Israel Deaconess Medical Center in Boston and a diplomate of the American Board of Psychiatry and Neurology. Her expertise includes Parkinson’s Disease and other movement disorders, patient selection for deep brain stimulation surgery and post-surgical management of deep brain stimulators, as well as the administration of Botox injections for dystonia, blepharospasm and hemifacial spasm. Pinky Agarwal, MD
Dr. Agarwal is a board certified neurologist, fellowship-trained in Movement Disorders at The Neurological Institute, Columbia-Presbyterian Medical Center, New York with Dr. Stanley Fahn. Her areas of interest include Parkinson’s disease, dystonia, essential tremor, restless leg syndrome, deep brain stimulation for dystonia, tremor and Parkinson’s disease, cerebellar ataxia, Huntington’s disease, Botox injections for spasticity and dystonia, tic/Tourette’s syndrome, and myoclonus. Sierra Farris, PA-C, MPAS
Neurostimulation Program Coordinator
Sierra M. Farris completed her physician assistant training at State University of New York at Stony Brook; earned a master’s degree in clinical neurology and is currently enrolled in the Philosophy Bioethics graduate program at Cleveland State University. Sierra cares for patients with movement disorders in a multidisciplinary setting including neuro-rehabilitation and neurosurgery. Marjie Kay Thomas, CMA
Marjie Kay Thomas graduated from Lake Washington Technical College in June 1995 as a Certified Medial Assistant. Marjie Kay has a twelve year history as a medical assistant specializing in movement disorders, geriatrics and primary care.
Teresa Wilson, MA
Teresa Wilson is a clinical research assistant at the Booth Gardner Parkinson’s Care Center. She obtained her medical assistant certificate from Lake Washington Technical Institute in 1996. She is currently taking pre-nursing classes at Shoreline Community College. Teresa’s areas of expertise include geriatrics, movement disorders and clinical research.

Martha Glisky, PhD
Dr. Martha Glisky is a clinical psychologist with a specialty in neuropsychology. She focuses on the evaluation and treatment of cognitive and emotional issues associated with neurologic disorders such as Parkinson's disease, Multiple Sclerosis, Alzheimer's disease, stroke, and brain injury. Jeff Shaw, PsyD
Jeffrey Shaw has been a staff Neuropsychologist with Evergreen Hospital since 2001. He works extensively with individuals diagnosed with Parkinson’s disease, Multiple Sclerosis, strokes, and varied neurological difficulties. Although Dr. Shaw specialized into the field of Neuropsychology (thorough evaluation of thinking skills), he also provides counseling and psychotherapy services. Ann Zylstra, PT
Ann Zylstra is the Supervisor of Evergreen’s out patient rehabilitation services and the Center’s lead physical therapist, with more than 21 years experience developing rehabilitation programs for patients with Parkinson’s and other neurological problems that affect balance and gait. Melissa Pei, PT
Melissa Pei is a licensed physical therapist and earned her Doctorate of Physical Therapy degree from the University of Washington in 2006. Evelyn Donascimento, PTA/L
Evelyn Donascimento is a physical therapy assistant and earned her Associate of Science in Physical Therapy at Miami Date Community College. Evelyn has twelve years experience working with persons diagnosed with Parkinson’s disease, multiple sclerosis, cerebrovascular accidents, traumatic brain injury, dementia and other neurological disorders.

Kay Ballen, OTR/L
Kay Ballen is the lead Occupational Therapist at the Booth Gardner Parkinson's Care Center. Kay has over 20 years experience working with neurologic diseases that affect the use of upper extremities, vision, cognition which include everyday activities such as eating, dressing, home chores, working, and driving.
Libby Winkler, OTR/L
Libby Winkler is an Occupational Therapist who graduated in 2001 from Nova Southeastern University and has been practicing occupational therapy for the past 5 years. Her areas of expertise include geriatrics and neurological disorders such as multiple sclerosis, stroke, brain injury, Parkinson’s Disease and other neurological disorders.
Shirley Glazer, SLP
Shirley Glazer, MS, CCC-Speech Language Pathologist, has a 15 year history of evaluation and treatment of neurological communication disorders in adults at Evergreen Hospital Medical Center. Her area of clinical interest is speech/voice disorders in Parkinson’s disease and other progressive disorders and is certified in the Lee Silverman Voice Treatment Method. Aside from speech/voice disorders in Parkinson’s, Shirley has extensive clinical experience in stroke rehabilitation, cognitive retraining, and motor speech disorders in an outpatient setting.
