Evergreen Home Health Services (EHHS) is a Medicare-certified home health agency, serving predominately the elderly in King and Snohomish Counties.

EHHS is dedicated to providing comprehensive, compassionate care to the community it serves, assisting residents to maximize their health, safety and independence in their place of residence.
PATIENT CARE DELIVERY MODEL
Multidisciplinary patient care is delivered to patients in their homes. A holistic, patient goal driven, problem oriented care plan is developed by the clinician, in partnership with the patient and designated caregivers. The care plan is derived from a thorough assessment of patient condition and environmental factors affecting a patient’s health and attainable outcomes. The multidisciplinary team supports patient driven goals and works collaboratively to achieve desired outcomes. Our case management model supports this approach, as well as providing care that is accessible and coordinated within the Evergreen Healthcare continuum.
TEAM MEMBERS
EHHS is comprised of four geographic teams, including team members of RN, LPN, PT, OT, SPT, MSW and home health aides, and are supported by their team clinical supervisor, clinical support specialists and clinical care coordinators.
SCHEDULING PRACTICES
EHHS office hours and primary work week is Monday through Friday 8 a.m. to 5 p.m. Intermittent visits are scheduled by nursing case managers and by each individual clinician, with the assistance of clinical support specialists and clinical care coordinators. Nurses work a rotation of weekends and holidays, roughly one weekend per month and two holidays per year. Physical therapists occasionally work Saturdays to admit new patients.
STAFF ORIENTATION
Extensive orientation is preceptor based and individualized, and is determined collaboratively by the new employee, orientation coordinator, preceptor and supervisor. EHHS is committed to providing an extensive orientation, so both the employee and organization are successful.
EDUCATIONAL AND DEVELOPMENT OPPORTUNITIES
EHHS strives to be the pre-eminent home health provider in the Puget Sound region. To ensure this goal, EHHS promotes high quality and applicable education in a variety of settings. Monthly discipline specific meetings, team meetings, hospital Grand Rounds and clinical classes taught by hospitalists are strongly recommended for all to attend. EHHS employs a Wound Care Specialist and Ostomy Specialist for consultation and support. Outside educational classes are also encouraged. The supportive atmosphere at EHHS encourages personal and professional growth. Many opportunities for advancement are available.
