PROFILE
Evergreen Hospice is a Medicare-certified and JCAHO accredited hospice agency providing palliative care to the terminally ill in King and Snohomish counties. Our services are provided in the patient’s home or place of residence as well as in our 15 bed hospice inpatient unit.
PATIENT CARE DELIVERY MODEL
Patient care is delivered via an interdisciplinary team with nursing and social work co-case management. The individualized plan of care is developed with physician, patient and family input, is focused on quality of life while addressing physical, emotional, and spiritual needs of patient and family throughout the end-of-life process, is goal driven and re-evaluated on a regular basis with all team members to assure coordinated services
TEAM MEMBERS
The hospice interdisciplinary team is comprised of RN and Social Work co-case managers with physician oversight; pharmacy, Certified Nurses Aide, Chaplain, and volunteer services are available as needed.
SCHEDULING PRACTICES
Office hours and primary work week is Monday through Friday 8 a.m. to 5 p.m. Regularly scheduled, intermittent visits are scheduled and provided to patients by all assigned team members with the assistance of our Patient Care Coordinator. On-Call nurses provide patient/family consultation evenings, nights and week-ends. Week-end visits and admissions are covered by case management staff hired on a seven days on/ seven days off schedule.
STAFF ORIENTATION
Staff orientation is preceptor based, individualized and determined collaboratively by the employee, preceptor, and supervisor. Evergreen Hospice is committed to an extensive orientation to further the success of both the employee and the organization.
EDUCATIONAL AND DEVELOPMENT OPPORTUNITIES
Evergreen Hospice strives to be the pre-eminent hospice provider in the Puget Sound region. To ensure this goal, Evergreen Hospice promotes high quality and applicable education in a variety of settings. Monthly discipline specific meetings, interdisciplinary team meetings, clinical classes taught by hospitalists are strongly recommended for all to attend. Outside classes are encouraged and supported for the development of personal and professional growth.
