PROFILE
Postpartum Care Center is an outpatient clinic with six private exam rooms for postpartum and or lactation. The clinic model of care is designed so that all families who deliver at Evergreen will be seen by our clinical staff when the infant is two to four days of age.
UNIT FOCUS
Our focus is to provide a high quality, comprehensive postpartum follow up appointment for both mother and baby. This service compliments our model of patient and family centered care. All plans are individualized to meet the needs of each mother or baby.
PATIENT CARE DELIVERY MODEL
RN postpartum specialists provide a comprehensive, full physical assessment of the mother and infant, noting conditions that are outside of defined normal limits ranges The RN will arrange appropriate follow-up and referrals as needed. Nurses also provide helpful tips regarding baby care, maternal care and emotional support for new families. Referrals are made to outside community programs as needed.
TEAM MEMBERS
The Postpartum Care Center staff is comprised of registered nurses who are especially trained as postpartum nurse specialists and are IBCLC certified or IBCLC eligible. We are supported by patient account representatives.
SCHEDULING PRACTICES
Appointments are for each family prior to their discharge from the Family Maternity Center. The appointment coincides with the baby’s day of age between two to four days of life, unless otherwise indicated by the primary care providers.
Clinic hours are Monday-Saturday, with first appointment at 8:30 with last appointment at 3:30, adjusting schedules to meet our family’s needs. Our appointments are scheduled for one hour. We are open most holidays.
STAFF ORIENTATION
All new hires or in house transfers receive orientation, which is preceptor based. Orientation is individualized as determined by the RN’s previous experience in mother/baby nursing with focus on breastfeeding.
EDUCATIONAL AND DEVELOPMENT OPPORTUNITIES
Participation in annual conferences and continuing education is ongoing. Evergreen Hospital has specific training for all staff in regards to lactation to meet Baby Friendly Status requirements.
